If you don't have a team name yet, please write "TBD."  Team names must be submitted to panoply@mtsinaishul.com by January 27th.

Each team member MUST make sure to register and pay independently. Please list team members when registering. Maximum 10 team members per team. If you do not have a team, please write "add me to a team." If you are still in the process of forming a team, please list the team members you have so far, or write "TBD."  
(Caveat: if you choose the Peerless Pilot sponsorship, your teammates don't need to register separately, but you will need to email panoply@mtsinaishul.com to confirm their registration.)
